I understand that parts or the whole of this event will be videotaped and that still photography will be used. I understand that by attending this event I hereby give my permission for my likeness and image to be used for any purposes that Fotos by Folletts deem appropriate and I hereby release Fotos by Folletts from any claim which could be made, including but not limited too, any claim in relation to my image and likeness.

All information submitted is considered confidential and private.
